摘要:
目的 运用新型眼前节光学相干断层成像仪CASIA2观察年龄相关性白内障患者晶状体倾斜及偏心的情况,并分析其影响因素。方法 本研究为横断面研究。选取2020年4月至2021年5月于天津医科大学眼科医院就诊并接受白内障手术的310例(310眼)年龄相关性白内障患者。运用CASIA2的晶状体分析模式测量晶状体倾斜与偏心,并对其作描述性分析。采用单因素线性回归分析及多因素线性回归分析患者晶状体倾斜及偏心与影响因素的关系。纳入分析的影响因素包括:年龄、糖尿病史、眼轴长度(AL)、中央角膜厚度(CCT)、房水深度(AD)、晶状体厚度(LT)、Alpha角及Kappa角。结果 纳入310例(310眼)白内障患者,年龄(68.8±8.5)岁,晶状体倾斜度为(4.97±1.59)°,偏心量为(0.21±0.12)mm。晶状体倾斜方向为颞下方,偏心方向为颞侧。其中,晶状体倾斜度>7°者占10.3%,偏心量>0.4 mm者占7.7%。单因素线性回归分析结果显示,晶状体倾斜度与晶状体偏心量、年龄、AL 、Alpha角及Kappa角均有相关性(均为P<0.05);而晶状体倾斜度与糖尿病史、CCT、AD、LT均无相关性(均为P>0.05)。晶状体偏心量与年龄、倾斜度、Alpha角及Kappa角均有相关性(均为P<0.05);而晶状体偏心量与糖尿病史、AL、CCT、AD、LT均无相关性(均为P>0.05)。多因素线性回归分析结果显示,晶状体倾斜度与晶状体偏心量、AL及Alpha角均有相关性(均为P<0.05);而晶状体倾斜度与年龄、糖尿病史、CCT、AD、LT及Kappa角均无相关性(均为P>0.05)。晶状体偏心量与倾斜度及AL均有相关性(均为P<0.05);而晶状体偏心量与年龄、糖尿病史、CCT、AD、LT、Alpha角及Kappa角均无相关性(均为P>0.05)。结论 年龄相关性白内障患者晶状体朝颞下倾斜、向颞侧偏心。在晶状体倾斜度较大的眼中,偏心程度更重。晶状体在短AL眼中及Alpha角大的眼中倾斜程度更重。白内障术前应综合考虑晶状体倾斜、偏心情况及AL、Alpha角等影响因素,合理选择功能型人工晶状体。
Abstract:
Objective To measure the crystalline lens tilt and decentration of patients with age-related cataracts and investigate their relationships with the influencing factors using a novel anterior segment optical coherence tomography CASIA2. Methods In this cross-sectional study, 310 age-related cataract patients (310 eyes) who underwent cataract surgery in Tianjin Medical University Eye Hospital from April 2020 to May 2021 were included. The crystalline lens tilt and decentration were quantified with CASIA2 under the lens analysis pattern, and descriptive analyses of the results were made. Univariate and multivariate linear regression analyses were used to evaluate the relationship between the crystalline lens tilt and decentration and their influencing factors. The assessed factors contained age, history of diabetes, axial length (AL), central corneal thickness (CCT), aqueous depth (AD), lens thickness (LT), angle alpha, and angle kappa. Results Of the 310 cataract patients (310 eyes) included, the age was (68.8±8.5) years, the crystalline lens tilt was (4.97±1.59)°, and the crystalline lens decentration was (0.21±0.12) mm. The crystalline lens tilted toward the inferotemporal direction and decentered toward the temporal direction. Among the patients, 10.3% had a crystalline lens tilt over 7°, and 7.7% had a crystalline lens decentration of more than 0.4 mm. The univariate linear regression analysis revealed that the crystalline lens tilt was correlated with crystalline lens decentration, age, AL, angle alpha and angle kappa (all P<0.05); it was not related to the history of diabetes, CCT, AD and LT (all P>0.05). The crystalline lens decentration was correlated with age, crystalline lens tilt, angle alpha and angle kappa (all P<0.05); it was not related to the history of diabetes, AL, CCT, AD and LT (all P>0.05). The multivariate linear regression analysis revealed that the crystalline lens tilt was correlated with crystalline lens decentration, AL and angle alpha (all P<0.05); it was not related to the age, history of diabetes, CCT, AD, LT and angle kappa (all P>0.05). The crystalline lens decentration was correlated with the crystalline lens tilt and AL (both P<0.05); it was not related to the age, history of diabetes, CCT, AD, LT, angle alpha and angle kappa (all P>0.05). Conclusion The crystalline lens axis of aged-related cataract patients tilts toward the inferotemporal direction and decenters toward the temporal direction. The crystalline lens decentration is severe in eyes with more significant tilt. The crystalline lens tilts severely in the eyes with short AL and large angle alpha. The crystalline lens tilt and decentration, AL and angle alpha should be considered before cataract surgery to select a functional intraocular lens reasonably.
