摘要:
年龄相关性黄斑变性(AMD)是一种随年龄增加而发病率逐渐升高并可导致中心视力下降、视物变形等的疾病,多发生于50岁以上老年人,双眼先后或同时发病,且视力损害逐渐加重。其中,色素上皮脱离(PED)在AMD中很常见。因各研究报道的结果不一致,本文通过参考详细定量参数,如高度、面积、宽度、最大线性直径,进行了全面文献回顾来评估PED与视力的关系。PED的存在可能与相对良好的视力相容,没有明显证据表明其存在或其形态的某些方面对视力有显著的影响。但PED的存在可能预示着需要更常规的治疗,如PED发展会引起视网膜色素上皮下脉络膜新生血管复合体的再激活和渗出,将导致长期视力丧失。因此需要标准化定义和分类研究来评估PED和视力之间的关系,本文就PED患者色素上皮形态与最佳矫正视力之间的关系作一简要综述。
Abstract:
Age-related macular degeneration (AMD) is a disease that increases with age and can lead to central vision loss and distorted vision. It is more common in people over 50 years of age, and occurs in both eyes successively or simultaneously, with gradually worse visual impairment. Pigment epithelial detachment (PED) is common in AMD. As the current research results are inconsistent, the relationship between PED and vision is evaluated through comprehensive literature review based on detailed quantitative parameters of PED, such as height, area, width, maximum linear diameter. The presence of PED may be compatible with relatively good vision, and there is no strong evidence that the presence of PED or some aspect of its form has a significant effect on vision. However, the presence of PED may indicate the need for more routine treatment, as the persistence of PED may cause reactivation and exudation of choroidal neovascularization (CNV) complex under retina pigment epithelium, leading to long-term vision loss. Therefore, standardized definitions and classifications are needed to evaluate the relationship between PED and vision. This review briefly reviews the relationship between pigment epithelial morphology and best corrected visual acuity(BCVA) in patients with PED.
