摘要:
目的 评估环曲面角膜塑形镜对伴较高角膜散光近视青少年患者的有效性和安全性。方法 收集2015年1月至6月在我院就诊的102例(189眼)伴-1.50~-3.50 D散光的青少年近视患者,随机分组,51例(92眼)配戴环曲面角膜塑形镜为试验组,51例(97眼)配戴框架眼镜为对照组,随访2 a,进行视力、等效球镜度数、眼轴长度等的比较,同时检测试验组患儿的角膜内皮细胞计数和中央角膜厚度。结果 试验组戴镜后1 d、1周、1个月、3个月、6个月、12个月、18个月、24个月的裸眼视力分别为4.21±0.36、4.78±0.21、4.99±0.08、5.02±0.12、5.03±0.08、4.98±0.13、4.95±0.09、4.86±0.08,戴镜后裸眼视力与戴镜前比较,差异均有统计学意义(均为P<0.05)。配戴2 a后,试验组等效球镜度数增加(-0.54±0.36)D,对照组增加(-1.74±0.43)D,两组相比差异有统计学意义(t=-17.988,P<0.01);眼轴长度试验组增加(0.41±0.18)mm,对照组增加(0.78±0.15)mm,两组比较差异有统计学意义(t=-13.820,P<0.01);试验组配戴前后角膜内皮细胞计数、中央角膜厚度差异均无统计学意义(均为P>0.05)。试验组未见严重并发症发生。结论 环曲面角膜塑形镜能有效缓解伴-1.50~-3.50 D较高角膜散光的青少年患者的近视进展。
Abstract:
Objective To assess the clinical efficacy and safety of the toric design orthokeratology treatment for myopic adolescents with moderate-to-high corneal astigmatism.Methods Together 102 patients (189 eyes) of myopic adolescents with -1.50 to -3.50 D corneal astigmatism were collected from Jan to Jun 2015 in our hospital,Random allocation was conducted,and 51 patients (92 eyes) wore the toric design orthokeratology lens as the experimental group,51 patients (97 eyes) wore frame glasses as the control group.Both groups were followed up for 2 years to observe the variations of vision,spherical equivalent,axial length.And finally the corneal endothelial cell count and the central corneal thickness were also measured in the experimental group.Results The uncorrected visual acuity of adolescents after wearing the toric design orthokeratology lens 1 day,1 week,1 month,3 months,6 months,12 months,18 months,24 months was 4.21±0.36,4.78±0.21,4.99±0.08,5.02±0.12,5.03±0.08,4.98±0.13,4.95±0.09,4.86±0.08,respectively.The uncorrected visual acuity improved significantly after lens wearing (P<0.05).After two years of wearing,the spherical equivalent of the experimental group increased by (-0.54±0.36)D,while the control group increased by (-1.74±0.43)D,and the difference between the two groups was statistically significant (t=-17.988,P<0.01).The axial length increased by (0.41±0.18)mm in the experimental group and (0.78±0.15)mm in the control group.The comparison difference between the two groups was statistically significant (t=-13.820,P<0.01).There was no significant difference in corneal endothelial cell count and central corneal thickness before and after wearing the toric design orthokeratology lens(P>0.05).No serious complications occurred in the experimental group.Conclusion Wearing the toric design orthokeratology lens can effectively delay the progression of myopia adolescents with -1.50 to -3.50 D corneal astigmatism.
