摘要:
目的 通过检测高度近视合并糖尿病患者玻璃体中血管内皮生长因子(vascularendotheialgrowthfactor,VEGF)和促红细胞生成素(erythropoietin,EPO)的浓度,探讨高度近视对糖尿病视网膜病变的影响。方法 选取待行玻璃体手术的视网膜脱离患者62例(62眼),其中对照组18眼,高度近视组16眼,增生型糖尿病视网膜病变(proliferativediabeticretinopathy,PDR) 组15眼,高度近视合并糖尿病组13眼,对玻璃体切割术中取出的玻璃体原液采用双抗体夹心酶联免疫吸附法(ELISA)进行VEGF、EPO含量的测定。结果 对照组玻璃体中VEGF及EPO浓度分别为(152.8±35.2)pg?mL-1、(74.3±16.5)U?L-1; 高度近视组为(81.3±17.5)pg?mL-1、(65.1±13.7)U?L-1;PDR组为(826.3±363.2)pg?mL-1、(649.8±267.9)U?L-1; 高度近视合并糖尿病组为(309.4±138.7)pg?mL-1、(156.2±56.7)U?L-1,高度近视合并糖尿病组玻璃体中VEGF、EPO浓度均低于PDR组,差异均有统计学意义(均有P<0.05)。结论 高度近视对糖尿病患者玻璃体中VEGF和EPO的表达具有抑制作用,VEGF和EPO低表达可能在高度近视阻滞糖尿病视网膜病变的发生发展中发挥作用。
Abstract:
Objective By measuring the levels of vascular endothelial growth factor ( VEGF) and erythropoietin ( EPO) in the vitreous body of diabetes mellitus with high myopia , to investigate the influence mecharusm of high myopia on diabetic retinopathy ( DR) . Methods Enzyme-linked immunosorbent assay ( ELISA) was used to detect the levels of VEGF and EPO in the vitreous body of 62 retinal detachment patients from vitrectomy in our hospital. There were 18 eyes in control group, 16 eyes in high myopia group , 15 eyes in proliferative diabetic retinopathy ( PDR) group and 13 eyes in high myopia with diabetes mellitus group. The differences of the values of VEGF and EPO between each group were compared and analyzed. Results The levels of VEGF and EPO in control group were ( 152. 8 + 35. 2) pg . mL -l and ( 74. 3 + 16. 5) U . L -l , respectively, in high myopia group were (81. 3 + 17. 5) pg . mL-l and (65. I + 13. 7) U . L-l ,respectively, in PDR group were ( 826. 3 + 363. 2) pg . mL -l and ( 649. 8 +267. 9 ) U . L -l , respectively ,in high myopia with diabetes mellitus group were ( 309. 4 +138. 7 ) pg . mL -l and ( 156. 2 +56. 7) U . L ’1 , respectively. The levels of VEGF and EPO in high myopia with diabetes mellitus group were significantly lower than PDR group ( all P < 0. 05 ) . Conclusion High myopia suppresses the expression of VEGF and EPO in the vitreous body of diabetic patients , the low expression of these factors may take Dart in the mechanisms of high myopia on preventing DR.
