摘要:
目的 探讨玻璃体内硅油填充对孔源性视网膜脱离患者黄斑中心凹下脉络膜厚度(SFCT)及眼轴的影响。
方法 收集2020年11月至2021年5月在遵义医科大学附属医院眼科确诊为孔源性视网膜脱离的患者31例(31眼)。本研究采用前后自身对照的病例系列研究方法,于术前,硅油填充术后1、3个月,硅油取出术后1、3、6个月测量患者黄斑中心凹厚度(CMT)、SFCT、眼轴长度以及眼压。通过重复测量方差分析和Spearman相关系数分析各指标不同时间点的差异及相关性。
结果 硅油填充术后1、3个月时患者CMT与术前比较,差异均无统计学意义(均为P>0.05);硅油取出术后1、3、6个月时患者CMT均厚于术前,差异均有统计学意义( 均为P<0.05)。硅油填充术后1、3个月及硅油取出术后1、3、6个月时患者SFCT均小于术前,差异均有统计学意义(均为P<0.05)。硅油填充术后1、3个月时患者眼轴长度大于术前,差异均有统计学意义(均为P<0.05),硅油取出术后1、3、6个月时患者眼轴长度均小于硅油填充术后3个月时,差异均有统计学意义(均为P<0.05)。硅油填充术后3个月时患者眼压高于术前,差异有统计学意义(P<0.001)。硅油填充术后3个月及硅油取出术后6个月,眼轴长度与SFCT均呈负相关(r=-0.669、-0.650,均为P<0.001),与眼压均呈正相关(r=0.426、0.521,均为P<0.05);SFCT与眼压无明显相关性(均为P>0.05)。
结论 孔源性视网膜脱离患者硅油填充期间SFCT变薄,眼轴变长,但硅油取出后眼轴可恢复到接近术前水平;硅油填充术后3个月及硅油取出术后6个月患者眼轴长度与SFCT呈负相关。
Abstract:
Objective To investigate the effect of intravitreal silicone oil (SO) tamponade on subfoveal choroidal thickness (SFCT) and axial length (AL) in patients with rhegmatogenous retinal detachment (RRD).
Methods Thirty-one patients (31 eyes) diagnosed with RRD at the Department of Ophthalmology of the Affiliated Hospital of Zunyi Medical University from November 2020 to May 2021 were included in the study. This study utilized a self-controlled case series method. Central macular thickness (CMT), SFCT, AL and intraocular pressure (IOP) of the patients were measured before surgery, one and three months after SO tamponade, one, three and six months after SO removal. The differences and correlations among the said indicators at different time points were analyzed using the repeated measures ANOVA and Spearman correlation coefficient.
Results Compared with the preoperative measurement, CMT showed no significant change one and three months after SO tamponade (both P>0.05) but increased significantly one, three and six months after SO removal (all P<0.05). SFCT decreased significantly one and three months after SO tamponade and one, three and six months after SO removal compared with that before operation (all P<0.05). AL at one and three months after SO tamponade was significantly higher than that before operation (both P<0.05), and AL at one, three and six months after SO removal was significantly lower than that at three months after SO tamponade (all P<0.05). IOP increased three months after SO tamponade compared with that before operation (P<0.001). AL was negatively correlated with SFCT (r=-0.669 and -0.650, both P<0.001) and positively correlated with IOP (r=0.426 and 0.521, both P<0.05) three months after SO tamponade and six months after SO removal; however, there was no significant correlation between SFCT and IOP (both P>0.05).
Conclusion SO tamponade makes RRD patients’ SFCT thinner and AL longer, but AL can be reduced to the preoperative level after SO removal. AL is negatively correlated with SFCT three months after SO tamponade and six months after SO removal.
