摘要:
目的 探讨不同浓度的人工泪液对合并中重度干眼的白内障患者角膜曲率测量和人工晶状体度数计算的影响。
方法 本研究共纳入合并中重度干眼的白内障患者80例80眼,依据患者所用人工泪液玻璃酸钠滴眼液的浓度将患者分为低浓度组和高浓度组各40例40眼。低浓度组患者采用1 g·L-1玻璃酸钠滴眼液滴眼;高浓度组患者采用3 g·L-1玻璃酸钠滴眼液滴眼。于用药前和用药后1、5、15 min测量受试眼平坦轴角膜曲率(K1),计算患者所需人工晶状体度数,对比其变化情况。
结果 用药后1 min和5 min,低浓度组受试眼K1均较用药前减小,差异均有统计学意义(均为P<0.05);用药后15 min,低浓度组受试眼K1与用药前比较,差异无统计学意义(P=0.556)。高浓度组受试眼用药前后K1变化情况与低浓度组相似。用药前两组受试眼K1差异无统计学意义(P>0.05)。用药后1 min,低浓度组受试眼K1较高浓度组升高(P<0.05)。用药后1 min和5 min,低浓度组受试眼人工晶状体度数均较用药前增大,差异均有统计学意义(均为P<0.05);用药后15 min,低浓度组受试眼人工晶状体度数与用药前比较,差异无统计学意义(P=0.160)。高浓度组受试眼人工晶状体度数变化情况与低浓度组相似。用药后1、5、15 min,两组受试眼人工晶状体度数差异均无统计学意义(均为P>0.05)。
结论 合并中重度干眼的白内障患者使用人工泪液短时间内会显著减小患眼K1,增大人工晶状体度数;建议使用人工泪液后应超过15 min再进行白内障术前生物测量。
Abstract:
Objective To investigate the effects of different concentrations of artificial tears on keratometric measurement and intraocular lens (IOL) power calculation of patients with cataracts combined with moderate to severe dry eyes.
Methods A total of 80 cataract patients (80 eyes) with moderate to severe dry eyes were enrolled. According to the concentration of sodium hyaluronate eye drops used, patients were randomly divided into the low-concentration group (40 eyes) and the high-concentration group (40 eyes). Patients in the low-concentration group were given 1 g·L-1 sodium hyaluronate eye drops, while those in the high-concentration group were administered 3 g·L-1 sodium hyaluronate eye drops. The corneal curvature in the flat axis (K1) was measured, and the IOL power required was calculated before, 1, 5, and 15 minutes after the administration. The changes were compared.
Results In the low-concentration group, at 1 and 5 minutes after the administration, K1 was significantly lower than that before administration (both P<0.05); at 15 minutes after the administration, there was no significant difference in K1 compared with that before administration (P=0.556). The changes of K1 in the high-concentration group were similar to those in the low-concentration group. There was no significant difference in K1 between the two groups before administration (P>0.05). At 1 minute after administration, K1 of the low-concentration group was significantly higher than that of the high-concentration group (P<0.05). In the low-concentration group, at 1 and 5 minutes after administration, the IOL power was significantly higher than that before administration (both P<0.05); at 15 minutes after administration, there was no significant difference in IOL power compared with that before administration (P=0.160). The changes of IOL power in the high-concentration group were similar to those in the low-concentration group. At 1, 5 and 15 minutes after administration, there was no significant difference in IOL power between the two groups (all P>0.05).
Conclusion The use of artificial tears on cataract patients with moderate to severe dry eyes can significantly decrease the K1 and increase the IOL power in a short time. It is recommended to perform the preoperative bio-measurement more than 15 minutes after the administration of artificial tears.
