摘要:
目的 利用眼前节光学相干断层扫描(optical coherence tomography,OCT)观察正常眼压性青光眼(normal tension glaucoma,NTG)患者白内障手术后前房角形态和眼压的变化。方法 共有106例患者纳入本研究,其中单纯年龄相关性白内障患者67例67眼为对照组、NTG合并白内障患者43例43眼为观察组。2组患者均行白内障超声乳化吸出联合人工晶状体植入术治疗。在术前和术后1个月、6个月测量眼压、前节OCT检查房角,自动计算四个象限(颞侧、鼻侧、上方和下方)的中央前房深度(anterior chamber depth,ACD)、前房宽度(anterior chamber width,ACW)、房角开放距离 (angle open distance,AOD)、小梁虹膜空间面积(trabecular iris area,TISA)、房角隐窝面积(angle recess area,ARA)。比较手术前后两组患者眼压、房角参数的变化。结果 观察组术前眼压为(13.2±2.9)mmHg(1 kPa=7.5 mmHg),术后1个月、6个月眼压分别为(10.5±3.0)mmHg和(10.7±2.8)mmHg。对照组术前眼压为(12.4±2.8)mmHg,术后1个月、6个月眼压分别为(11.6±2.5)mmHg、(12.0±2.8)mmHg。观察组手术前后眼压比较差异有统计学意义(P<0.001)。两组患者手术后房角参数AOD、ARA和TISA均增加,房角参数的变化(颞侧ΔAOD500、ΔTISA500及鼻侧ΔAOD500、ΔARA500)与术后眼压变化呈线性相关。结论 白内障手术后可能改善房角参数,降低NTG患者的眼压。
Abstract:
Objective To investigate the changes of anterior chamber angle morphology and intraocular pressure (IOP) after cataract surgery in patients with normal tension glaucoma (NTG) using anterior segment optical coherence tomography (OCT).Methods Totally 106 cataract patients were enrolled in the study,including 67 eyes of 67 simple age-related cataract patients (control group) and 43 eyes of 43 NTG combined with cataract patients (observation group).All patients were treated with phacoemulsification and intraocular lens (IOL) implantation.Before surgery,at 1 month and 6 months after surgery,IOP was measured,and chamber angle was measured by anterior segment OCT to automatically calculate the central anterior chamber depth (ACD) and anterior chamber width (ACW) of the four quadrants (temples,nose,upper and lower),angle open distance(AOD),trabecular iris area (TISA) and angle recess area (ARA).The changes of IOP and angle parameters were compared between the two groups before and after surgery.Results In observation group,IOP were (13.2±2.9)mmHg before surgery,(10.5±3.0)mmHg and (10.7±2.8)mmHg at 1 month and 6 months after surgery.In control group,IOP were (12.4±2.8)mmHg before surgery,(11.6±2.5)mmHg and (12.0±2.8)mmHg at 1 month and 6 months after surgery.Statistical difference was found in IOP before and after surgery in observation group (P<0.001).The angle parameters (ΔAOD500 and ΔTISA500 at temporal side;ΔAOD500 and ΔARA500 at nasal side) were linearly correlated with IOP changes after surgery.Conclusion Chamber angle parameters get improvement and IOP decreases in NTG patients after cataract surgery.
