摘要:
目的 超声乳化白内障吸出联合人工晶状体植入术与可植入式角膜接触镜(implantable collamer lens,ICL)植入术后角膜曲率及散光的变化,并进行比较,为屈光性白内障手术视觉质量的提高提供理论指导。方法 收集我院行超声乳化白内障吸出联合人工晶状体植入术的患者34例(52眼)为A组,同期行ICL植入术的患者26例(52眼)为B组,两组患者手术前后使用Pentacam眼前节分析系统检测角膜前后表面曲率、角膜散光。比较两组患者术前和术后3个月的角膜前表面曲率、角膜后表面曲率、角膜前表面散光、角膜后表面散光及角膜总屈光力与总散光,比较两组患者手术前后各数值变化值的差异。结果 A组患者手术前后角膜前表面曲率及散光差异均无统计学意义(P=0.734、0.427),角膜后表面曲率、角膜后表面散光差异均有统计学意义(P=0.022、0.000)。B组患者手术前后角膜前表面曲率及前表面散光差异均无统计学意义(P=0.475、0.446),角膜后表面曲率及后表面散光差异均有统计学意义(P<0.001,P=0.016)。A组与B组患者手术前后角膜前表面曲率差值、角膜前表面散光差值差异均无统计学意义(P=0.534、0.841)。A组患者手术前后角膜后表面曲率差值、角膜后表面散光差值均大于B组,差异均有统计学意义(P=0.001、0.022)。A组与B组手术前后角膜总散光差异有统计学意义(P<0.05)。结论 白内障超声乳化术与ICL植入术对角膜前表面曲率及前表面散光的影响较小,但是对角膜后表面曲率及后表面散光的影响较大,且白内障超声乳化术较ICL植入术对角膜后表面曲率及后表面散光的影响更大。
Abstract:
Objective To investigate the changes of corneal curvature and astigmatism after phacoemulsification combined with IOL implantation,and compare the results with those after implantable collamer lens (ICL) implantation,so as to provide further theoretical guidance for the improvement of visual quality after refractive cataract surgery.Methods There were 34 patients (52 eyes) in group A,who received phacoemulsification combined with IOL implantation in our hospital,and another 26 patients (52 eyes) in group B received ICL implantation at the same period.For patients in both two groups,visual acuity was detected,and corneal anterior and posterior surface curvature and astigmatism were measured by Pentacam Anterior Segment Analysis System before and after surgery.Corneal anterior and posterior surface curvature and astigmatism were respectively compared between the two groups before surgery and three months after surgery.The difference of each value was compared for both two groups.Results There was no significant difference in corneal anterior surface curvature or astigmatism before and after surgery in group A (P=0.734,P=0.427),but significant differences were found in corneal posterior surface curvature (P=0.022) and corneal posterior surface astigmatism (P=0.000).There was no significant difference in corneal anterior surface curvature or astigmatism in group B before and after surgery (P=0.475,P=0.446),but statistical differences were found in corneal posterior surface curvature and astigmatism (P<0.001,P=0.016).The differences in corneal anterior surface curvature and astigmatism before and after surgery had no statistical significance between group A and group B (P=0.534,P=0.841).The differences of corneal posterior surface curvature and astigmatism before and after surgery in group A were statistically greater than those in group B (P=0.001,P=0.022).The difference of corneal astigmatism before and after surgery had statistical significance between group A and group B (all P<0.05).Conclusion Both phacoemulsification and ICL implantation have less effect on corneal anterior surface curvature and astigmatism,but more effect on corneal posterior surface curvature and astigmatism.Phacoemulsification has greater effect on posterior surface curvature and astigmatism than ICL implantation.
