摘要:
目的 利用多焦视网膜电图(mf-ERG)探讨白内障超声乳化术后重度非增生型糖尿病视网膜病变患者视网膜功能的变化。方法 选取2015年6月至2016年6月在苏州大学附属第二医院眼科中心接受常规白内障超声乳化联合人工晶状体植入术的重度非增生型糖尿病视网膜病变患者36例36眼。糖尿病病程(10.3±2.9)a;术前视力0.10±0.06。所有患者术眼术前1周及术后6个月采用mf-ERG进行检查。检测项目包括术前和术后视网膜后极部各环和四个象限P1波和N1波潜伏期和振幅密度。结果 术后6个月,患者在0、1、2环的P1波潜伏期均较术前延长(均为P<0.05);3、4环P1波潜伏期均较术前变化不大(均为P>0.05);0、1环N1波潜伏期较术前延长(均为P<0.05);2、3、4环N1波潜伏期变化不大(均为P>0.05)。术后6个月,患者在0、1环的P1波、N1波振幅密度均较术前减小(均为P<0.05);2、3、4环P1波、N1波振幅密度变化不大(均为P>0.05)。术后6个月,患者鼻上象限P1波潜伏期较术前延长(P<0.05);鼻下、颞上、颞下象限P1波潜伏期变化不大(均为 P>0.05);四个象限N1波潜伏期均较术前变化不大(均为P>0.05)。术后6个月,患者四个象限P1波和N1波振幅密度较术前均变化不大(均为P>0.05)。结论 白内障超声乳化术后重度非增生型糖尿病视网膜病变患者病变会进一步加重,mf-ERG能客观定量地评定视网膜功能的变化。
Abstract:
Objective To investigate the changes in retinal function of patients with severe non-proliferative diabetic retinopathy after phacoemulsification surgery using multifocal electroretinogram (mf-ERG).Methods Thirty-six eyes of 36 cataract patients with severe non-proliferative diabetic retinopathy from the Department of Ophthalmology,the Second Affiliated to Sowchow University who were underwent phacoemulsification surgery between June 2015 and June 2016 were collected.All surgical eyes of patients received mf-ERG examination one week before surgery and six months after surgery,respectively.The average of diabetic duration was (10.3±2.9)years;the average visual acuity before surgery was 0.10±0.06.The test contained the latency and amplitude densities of P1 and N1 waves in different rings and four quadrants of retinal posterior pole between preoperative and postoperative measurements.Results Six months later,the latency of P1 wave prolonged significantly from the ring zero to two after operation compared with before operation(all P<0.05);no significant changes were shown in the latency of P1 wave from the ring three to four before and after operation (all P>0.05);the latency of N1 wave prolonged obviously from the ring zero to one after operation compared with before operation(all P<0.05);no significant differences were shown in the postoperative and preopertive latency of N1 wave from the ring two to four(all P>0.05).Six months later,the postoperative amplitude densities of P1 and N1 wave were significantly attenuated from the ring zero to one compared with before operation(all P<0.05);there was no significant changes in the amplitude densities of P1 and N1 wave from the ring two to four before and after operation(all P>0.05).Six months later,the latency of P1 wave prolonged in the superonasal quadrant after postoperation compared with before operation(P<0.05);no significant differences were shown in the latency of P1 wave in the inferonasal,superotemporal,lower temporal quadrants and the latency of N1 wave in four quadrants after operation(all P>0.05).In addition,no significant differences were shown in the amplitude densities of N1 and P1 waves in four quadrants before and after operation(all P>0.05).Conclusion The phacoemulsification surgery unfortunately accelerate the progress of severe non-proliferative diabetic retinopathy,and the changes in retinal function can be detected objectively and quantitatively by the mf-ERG.
