摘要:
目的 对比人工泪液与生理盐水在泪道磁共振水成像(magnetic resonance hydrography,MRH)中的优缺点。方法 招募健康志愿者33例(健康组)及泪道梗阻志愿者24例(患者组)行MRH检查,于健康组双眼、患者组患侧眼结膜囊先滴入人工泪液(A组),第2天换用生理盐水(B组),而后均运用MRH技术行泪道扫描并进行图像后处理,对受检者应用两种滴眼液所得MRH图像质量及泪道长度进行对比分析。结果 所有志愿者的泪囊、鼻泪管在滴用两种滴眼液后均有不同程度的MRH显像。图像质量方面,除部分受试者应用两种对比剂所得的图像质量近似外,其余健康组17例34侧、患者组16例18侧的A组泪道图像质量均优于B组;所有志愿者MRH图像中A组的泪道长度均不同程度长于B组,配对t检验示差异有统计学意义(P<0.05)。10例志愿者使用生理盐水后主诉有一般不适,所有志愿者使用人工泪液时均未诉有不适感。结论 应用人工泪液的泪道MRH检查是一种无创、无辐射、可执行、舒适度较高的泪道检查技术,其图像质量、舒适度优于以往的生理盐水成像,可为泪道疾患的诊治提供更加直观、准确的解剖及功能信息。
Abstract:
Objective To compare the advantages and disadvantages of artificial tears and physiological saline in magnetic resonance hydrography (MRH) application.Methods Total 33 healthy volunteers and 24 patients with lacrimal duct obstruction underwent MRH examination.The image post-processing of the result data from the relevant MR scanning,by MRH technology,after both sides (health group) or affected side (patients group) of the conjunctival sac instillation of artificial tears(group A)and physiological saline(group B).And then,the image quality and lacrimal duct length between the application of two kinds of eye drops would be compared by senior doctors of our department.Results The lacrimal sac and nasolacrimal duct of all examinees were varyingly displayed by MRH.Images of 17 volunteers and 16 patients obtained by using artificial tears were superior to those using physiological saline;and of the remaining ones,there was no significant difference.After using two kinds of eye drops,the developed lacrimal duct in MRH images of group A were longer than those of group B by different degrees,with statistical significance by matched t-test (P<0.05).After using of physiological saline injection,10 examinees felt general orbital discomfort,and there was no orbital discomfort after using of artificial tears.Conclusion By using artificial tears,MRH examination is a kind of lacrimal duct imaging technique with non invasive,non radiation and feasible,comfortable.The image quality obtained and the degree of comfort by using artificial tears is better than that of physiological saline injection in the past.It could provide more direct and accurate anatomical and functional information for the diagnosis and treatment of lacrimal duct diseases.
