摘要:
目的　观察超声乳化联合玻璃体内注射雷珠单抗治疗糖尿病性白内障的临床效果。方法　选取２０１３年５月至２０１５年５月我院收治的８６例（１２７眼）糖尿病性白内障患者作为研究对象，根据治疗方法的不同将患者分为观察组（４３例，６４眼）和对照组（４３例，６３眼），均接受白内障超声乳化摘出术，观察组同时给予玻璃体内注射雷珠单抗。术后１周、１个月、３个月检查患者手术前后视力、视网膜黄斑区厚度，同时观察记录２组术后并发症情况。结果　２组术前视力差异无统计学意义（Ｐ＞０．０５）；２组术后各时间点视力均较术前提高，差异均有统计学意义（均为Ｐ＜０．０５）；２组组间术后各时间点视力差异均有统计学意义（均为Ｐ＜０．０５）。２组患者术前视网膜黄斑区厚度差异无统计学意义（Ｐ＞０．０５）；术后２组患者视网膜黄斑区厚度均较术前有不同程度增加，但观察组术后不同时间点视网膜黄斑区厚度与术前比较，差异均无统计学意义（均为Ｐ＞０．０５），而对照组术后不同时间点视网膜黄斑区厚度与术前比较，差异均有统计学意义（均为Ｐ＜０．０５）；２组组间术后各时间点视网膜黄斑区厚度比较，差异均有统计学意义（均为Ｐ＜０．０５）。２组并发症发生率差异无统计学意义（Ｐ＝０．４９６１）。结论　糖尿病性白内障患者行白内障超声乳化摘出联合玻璃体内注射雷珠单抗安全有效，能有效预防术后黄斑水肿，提高患者术后视力。
Abstract:
Objective To evaluate the clinical effects of phacoemulsification combined with intravitreal injection of lucentis for diabetic cataract. Methods A total of 86 patients ( 127 eyes) with diabetic cataract were analyzed retrospectively. According to the type of their previous treatment.the patients were divided into observation group (43 cases,64 eyes) and control group (43 eyes,63 eyes). The patients of two groups all received phacoemulsification . and moreover.the observation group was intravitreal injected lucentis. The preoperative and postoperative visual acuity, retinal macular thickness were detected before and I week.l month.3 months after operation, and the complication was also observed. Results There was no statistical difference in preoperative visual acuity between two groups ( P > 0 . 05 ) ; The postoperative visual acuity in two groups were all increased. there were statistical differences compared with pre-operation ( all P < 0. 05 ) ;There were statistical differences in postoperative visual acuity at different time between two groups ( all P < 0. 05 ). There was no statistical difference in preoperative retinal macular thickness between two groups ( P > 0. 05 ) ; The postoperative visual acuity in two groups were all increased, but there was statistical differences only in control group at different time compared with pre-operation ( all P < 0. 05 ) ; There were statistical differences in postoperative retinal macular thickness at different time between two groups ( all P < 0. 05 ) . There was no statistical difference in the incidence of complications between two groups (P > 0. 05) . Conclusion Phacoemulsification combined with intravitreal injection of lucentis for diabetic cataract is safe and effective , can prevent the macula edema and improve the visual acuity.
