摘要:
目的 评估急性视网膜坏死(ARN)患者的临床特征以及中性粒细胞与淋巴细胞比值(NLR)在ARN发生发展过程中的作用。方法 回顾性分析2017年7月至2021年10月于北京朝阳医院就诊的ARN患者27例(27眼)为病例组,选择性别、年龄完全匹配的同时期健康体检者为对照组,分析两组受试者中性粒细胞计数、淋巴细胞计数、NLR和血小板平均体积的差异;评估病例组患者ARN的临床特征,并进行不同性别亚组间比较,分析NLR对视网膜脱离的预测价值。结果 ARN患者年龄中位数为50岁,男15例(55.6%),女12例(44.4%)。所有患者中17例(63.0%)为水痘带状疱疹病毒感染,1例(3.7%)为巨细胞病毒感染,还有9例(33.3%)患者未行感染病毒类型检测。所有患者中9例(33.3%)发生视网膜脱离。男性亚组患者血小板平均体积明显高于女性(P=0.038),男性亚组和女性亚组间其他基线资料差异均无统计学意义(均为P>0.05)。病例组和对照组受试者中性粒细胞计数(P=0.009)、淋巴细胞计数(P=0.025)和NLR(P=0.002)差异均有统计学意义。病例组和对照组受试者血小板平均体积差异无统计学意义(P=0.283)。Logistic回归分析发现中性粒细胞计数(P=0.523)、淋巴细胞计数(P=0.383)、NLR(P=0.765)、血小板平均体积(P=0.580)与ARN患者视网膜脱离均不相关。结论 NLR与ARN的发病有关,尚未发现NLR与ARN伴视网膜脱离有关。
Abstract:
Objective To investigate the clinical features of acute retinal necrosis (ARN) and the role of neutrophil-to-lymphocyte ratio (NLR) in the development of ARN. Methods A total of 27 ARN patients (eyes) admitted to Beijing Chaoyang Hospital from July 2017 to October 2021 were retrospectively collected and divided into the ARN group. The sex and age-matched healthy individuals during the same period were classified into the control group. The differences in neutrophil count (NC), lymphocyte count (LC), NLR, and mean platelet volume (MPV) between the two groups were analyzed. The clinical features of ARN patients were evaluated and compared among sub-groups to explore the predictive value of NLR for retinal detachment. Results The median age of ARN patients [15 males (55.6%) and 12 females (44.4%)] was 50 years. Among the patients, 17 (63%) were infected with varicella-zoster virus, 1 (3.7%) was infected with cytomegalovirus, and 9 (33.3%) didn’t receive the infection test. Retinal detachment occurred in 9 patients (33.3%). The MPV of male patients was significantly higher than that of female patients (P=0.038), while other baseline data showed no significant difference between the male and female patients (all P>0.05). There were significant differences in NC (P=0.009), LC (P=0.025) and NLR (P=0.002), but no significant difference in MPV (P=0.283) between ARN and control groups. The logistic regression analysis results showed that NC (P=0.523), LC (P=0.383), NLR (P=0.765) and MPV (P=0.58) were not associated with retinal detachment of ARN patients. Conclusion NLR is associated with the occurrence of ARN; however, it has not been found that NLR is associated with retinal detachment, a complication of ARN.
