目的 探讨Pentacam眼前节分析仪光密度测定法定量评估白内障手术后轻度角膜水肿的临床应用价值。方法 134例155眼白内障患者接受白内障手术,在术前及术后1 d、3 d和7 d使用Pentacam进行角膜光密度测量,观察手术前后不同部分角膜光密度值的连续变化,同时记录角膜厚度的变化,分析二者之间的相关性。结果 上皮层及前弹力层中心2 mm区域角膜光密度值从手术前的18.12±1.76增加到术后1 d的21.03±3.84(P<0.001)和术后3 d的19.90±2.46(P=0.018),但在术后7 d 恢复到19.44±1.58(P=0.131)。手术前角膜厚度为(540.1±32.4)μm,在术后1 d增加到(588.3±53.9)μm(P=0.001),但在术后3 d恢复到(567.8±29.9)μm(P=0.097)。上皮层中心2 mm 区域角膜光密度值与角膜厚度呈正相关(r=0.13,P=0.003),基质层、内皮层光密度值及角膜全层平均光密度值与角膜厚度均未见相关性。结论 Pentacam光密度测定法可定量评估裂隙灯检查不易发现的轻度角膜水肿。
Abstract:
Objective To explore the clinical application of Pentacam optical density methods for quantitative evaluation for mild corneal edema after cataract surgery. Methods All 155 eyes of 134 patients underwent cataract surgery. The corneal optical density was measured with Pentacam before operation, 1 day, 3 days and 7 days after operation. The continuous change in corneal optical density before and after the operation was observed, and the change in corneal thickness was recorded. Finally, the correlation of corneal optical density with corneal thickness was analyzed. Results The corneal optical density value in the 2mm area of center epithelial layer and center anterior elastic layer increased from 18.12±1.76 before cataract surgery to 21.03±3.84 on the first postoperative day (P<0.001) and 19.90±2.46 on the third day (P=0.018), but recovered to 19.44±1.58 on the seventh day (P=0.131). The corneal thickness was (540.1±32.4) μm before surgery, which increased to (588.3±53.9) μm on the first day (P=0.001), but returned to (567.8±29.9) μm (P=0.097) on the third day. The corneal optical density value in the 2mm area of center epithelial layer is positively correlated with the corneal thickness (correlation coefficient = 0.130, P=0.003). Conclusion Pentacam optical density methodscan quantitatively evaluate mild corneal edema which is not easy to be found by slit lamp examination.
