摘要:
目的 探讨逆向透明角巩膜隧道切口及鱼钩系线法联合小切口折叠人工晶状体睫状沟缝线固定术的安全性、有效性和手术技巧。方法 回顾性分析我院2014年2月至2017年4月行人工晶状体睫状沟固定的患者26例(27眼)的临床资料。切口采用2点、8点钟位角膜缘对称位、2.8 mm逆向透明角巩膜隧道切口,隧道长度为角膜缘后1.5 mm。将三片式折叠人工晶状体使用双针双线聚丙烯缝线用鱼钩系线法分别固定两襻,利用推注器通过2.8 mm透明角膜切口注入后房,人工晶状体襻预置缝线分别在两个逆向透明角巩膜隧道切口内结扎掩埋。术后随访6个月,观察术中及术后并发症,并比较术前术后视力、散光,使用OPD-ScanⅢ观察人工晶状体位置。结果 26例(27眼)患者手术均顺利完成,术中无出血,术后无缝线脱落或暴露、人工晶状体无明显倾斜或移位等。术后最佳矫正视力:2眼1.0,10眼0.6~0.8,12眼0.4~0.5,3眼0.3。术后散光度为(0.76±0.23)D,与术前比较差异无统计学意义(P=0.432)。术后1周、1个月、3个月、6个月人工晶状体倾斜Tilt值分别为0.42±0.13、0.45±0.15、0.48±0.16、0.49±0.19,各时间点比较差异均无统计学意义(均为P>0.05)。结论 运用逆向透明角巩膜隧道切口损伤轻,缝线掩埋良好,术源性散光小;鱼钩系线法可使人工晶状体位置稳定,安全性高,是对人工晶状体睫状沟缝线固定术较好的改良方法。
Abstract:
Objective  To investigate the safety, efficacy and technique of reverse transparent keroscleral tunnel incision with fishhook lines combined with intraocular lens (IOL) implantation and ciliary sulcus fixation. Methods A retrospective review was conducted on clinical data of 26 patients (27 eyes) who received IOL implantation and ciliary sulcus fixation from February 2014 to April 2017. The incision tunnel was performed at 2 o’clock and 8 o’clock in the symmetrical position of corneal limbus and the length of the tunnel was 1.5 mm after the limbus. A 2.8 mm reverse transparent keroscleral tunnel incision was made.
