摘要:
目的　探讨频域光相干断层扫描（ｏｐｔｉｃａｌｃｏｈｅｒｅｎｃｅｔｏｍｏｇｒａｐｈｙ，ＯＣＴ）在早期诊断周边视网膜裂孔中的临床应用价值。方法　对我院２６例（２６眼）经ＯＣＴ完全确诊为早期周边视网膜裂孔的２６例患者资料进行回顾性分析，均为单眼发生，所有患者ＯＣＴ检查前均经散瞳后前置镜、三面镜或双目检眼镜检查，对周边疑似裂孔的位置进行画图、定位，然后采用ＯＣＴ进行检查，观察ＯＣＴ对早期视网膜周边裂孔诊断的特异性、准确性。结果　２６眼患者中１３眼ＯＣＴ提示为牵拉性裂孔，其中９眼显示牵拉条索与裂孔的一端相连，４眼显示孔的盖已经完全脱离于孔的上缘；７眼ＯＣＴ提示为干性裂孔；６眼ＯＣＴ提示视网膜周边裂孔并伴有局限性视网膜脱离，４眼提示为牵拉性视网膜脱离，２眼裂孔周围视网膜局限性脱离，未发现牵拉组织形成。结论　ＯＣＴ在早期视网膜周边裂孔的诊断、判断裂孔的性质、指导临床治疗方面具有重要意义。
Abstract:
Objective To investigate the clinical values of optical coherence tomography ( OCT) in the early diagnosis of peripheral retinal holes. Methods Twenty-six eyes in 26 cases in our hospital with peripheral retinal holes that were early diagnosed by OCT were analyzed retrospectively. The position of suspected peripheral retinal holes of all patients were examined by slit lamp ophthalmoscope , trihedral reflector or indirect ophthalmoscope though dilated pupil and then checked by OCT. The diagnostic specificity and accuracy of OCT in early peripheral retinal holes were evaluated. Results OCT showed tractional holes in 13 eyes from the 26 eyes. Among 13 eyes,9 cases showed tractional cord connected with one side of the holes , and 4 eyes showed the lid of the hole had totally separated from the top edge. 7 eyes demonstrated dry holes. 6 eyes showed peripheral retinal holes with partial retinal detachment. Among 6 eyes,4 eyes showed tractional retinal detachment and 2 eyes with partial retinal detachment showed no tractional structure. Conclusion OCT has important meanings in early diagnosis of peripheral retinal holes , assess of the hole , and clinical treatment.
