摘要:
目的 探讨妊娠期并发高血压及(或)高血糖与早产儿视网膜病变的关系。方法 回顾分析我院2014年3月至2015年2月早产儿或低出生体质量儿早产儿视网膜病变筛查情况及其孕母临床资料。对妊娠期高血压及妊娠期高血糖与早产儿视网膜病变发病的相关性进行分析。结果 收集451例早产儿或低出生体质量儿中早产儿视网膜病变患儿53例(11.75%),患病组与非患病组出生体质量、吸氧史、孕母年龄、孕母高血压及高血糖患病情况差异均有统计学意义(均为P<0.05)。早产儿视网膜病变与孕母患高血压情况呈正相关关系(rs=0.390,P<0.01),与孕母患高血糖情况亦正相关(rs=0.284,P<0.01)。妊娠期高血糖发生相对于妊娠期高血压的相对危险度Rr=2.03;妊娠期高血压发生相对于妊娠期血糖异常的相对危险度Rr=1.55。妊娠期高血压的发生与妊娠期高血糖疾病的发生亦具有相关性(χ2=24.37,P<0.01)。结论 妊娠期高血压与妊娠期高血糖与早产儿视网膜病变的发生相关,且妊娠期高血压与妊娠期高血糖的发病具有相关性。
Abstract:
Objective To analyze the relationship between maternal hypertension,hyperglycemia and retinopathy of prematurity ( ROP) . Methods Premature infants or low birth weight infants in neonatal intensive care unit of our hospital between March 2014 and February 2015 were included. Clinical data of ROP incidence of these infants and relevant condition of their mother were collected and analyzed. Furthermore , correlation analysis in-depth was performed about maternal hypertension , hyperglycemia and ROP,respectively. Results Fifty-three patients were detected with ROP in 451 infants ( 11. 75% ). The difference in birth weight, oxygen inhalation. maternal age,maternal hypertension and maternal hyperglycemia between ROP group and nonROP group were statistically significant ( all P < 0. 05 ) . Analysis in-depth showed that ROP was sigruficant positive linear related with maternal hypertension ( rs = 0. 390 ,P < 0. 01 ) and maternal hyperglycemia ( rS = 0. 284 ,P < 0. 01 ) . The relative risk degree ( Rr) of matemal hyperglycemia to matemal hypertension was 2. 03 ; Rr of matemal hypertension to matemal hyperglycemia was I . 5 5 . The relationship of maternal hypertension and maternal hyperglycenua was significant and positive (X2 = 24. 37 ,P < 0. 01) . Conclusion Maternal hypertension and hyperglycemia are all related positively with ROP. Moreover.maternal hypertension and maternal hyperglycemia are related mutually.
