摘要:
 目的 探讨飞秒激光制瓣LASIK治疗近视散光的准确性及影响因素。方法 收集行飞秒LASIK手术患者76例(127眼),等效球镜度数≤ -9.00D,柱镜度数≥ -0.50D,按术前散光度数分为中低度散光组(-0.50~-2.00D)51例(93眼)、高度散光组(-2.25~-5.00D)25例(34眼)。收集并比较手术前后视力、屈光度、散光及散光轴位变化,使用CRAVY矢量分析法将散光进行相关分析,SPSS18.0软件进行统计学分析。结果 术后中低度散光组裸眼视力达到或超过术前最佳矫正视力85眼(91%);术后残余散光在±0.5D以内82眼(88.2%),±1.0D以内93眼(100%);高度散光组裸眼视力达到或超过术前最佳矫正视力30眼(89%)。术后残余散光在±0.5D、±1.0D以内分别为26眼(76.5%)、33眼(97.0%)。散光进行CRAVY矢量分解分析,术后中低度散光组J0为-0.068±0.152、J45为0.001±0.123,高度散光组J0为-0.019±0.184、J45为0.018±0.212,两组之间差异均无统计学意义(均为P>0.05)。结论 运用飞秒激光制瓣LASIK手术对近视散光的治疗安全、有效,且准确性高。
Abstract:
 Objective To study the influence factors and accuracy of femtosecond laser in situ keratonuleusis ( FS-LASIK) for myopic astigmatism. Methods A retrospective study was carried out in 76 patients ( 127 eyes) who had undergone FS-LASIK. The patients ’ spherical equivalent ( SE) was≤ - 9. 00 D , and astigmatism was≥ - 0. 50 D. The patients were divided into low-to-moderate astigmatism group ( 51 cases, 93 eyes, -0. 50 - - 2. 00 D) and high astigmatism group ( 25 cases,34 eyes, - 2. 25 -5. 00 D) according to the preoperative astigmatism. The preoperative and postoperative vision , diopter, astigmatism and axis of astigmatism were collected and compared. CRAVY vector analysis was used for correlation analysis , and SPSS18. 0 and Excel were used for statistical analysis. Results In the low-to-moderate astigmatism group, the postoperative uncorrected visual acuity of 85 eyes (91% ) reached or exceeded the preoperative best corrected visual acuity , the postoperative residual astigmatism of 82 eyes ( 88.1% ) were within +0. 5 D,93 eyes (100% ) were within + 1. 0 D. In high astigmatism group ,the postoperative uncorrected visual acuity of 30 eyes (8g% ) reached or exceeded the preoperative best corrected visual acuity ,the postoperative residual astigmatism of 26 eyes (76. 5% ) were within +0. 5 D and 33 eyes (97. 0% ) were within + 1.0 D. After the operation,the differences of JO ( - 0. 068 +0. 152. - 0. 019 +0. 184 , respectively) and J45 ( 0. 001 + 0. 123 .0. 018 + 0. 212 ,respectively) between the two groups were not statistically significant ( all P > 0. 05 ) . Conclusion FS-LASIK is safe and effective for myopic asti~natism with high accuracy.
