摘要:
 目的 应用光学相干断层扫描(opticalcoherencetomography,OCT)对弱视及正常儿童黄斑参数进行测量及相关性分析,为探索弱视发病的外周机制提供依据。方法 屈光参差性及屈光不正性弱视组患者60例(80眼)(弱视组)及正常对照组80例(80眼)(正常组),应用光学生物测量仪(IOLMaster)和OCT仪检测两组的眼轴长度及各黄斑参数,用SPSS13.0统计软件分析两组的差异。结果 弱视组与正常组相比,黄斑厚度在中心凹(242.41±22.77)μm及外环区更厚(P=0.048、0.000、0.005、0.000、0.001),而内环区四个象限黄斑厚度、平均黄斑厚度(282.98±23.05)μm和体积(9.96±0.73)mm3的差异均无统计学意义(均为P>0.05)。弱视组各黄斑参数无性别差异(均为P>0.05),而正常组男生与女生相比,黄斑厚度在内环鼻侧、下方、颞侧更厚(P=0.03、0.01、0.02)。不同弱视程度各黄斑参数的差异均无统计学意义(均为P>0.05)。弱视组内环下方的黄斑厚度与矫正视力具有相关性(r=0.267,P=0.017),外环鼻侧与屈光度、眼轴长度存在相关性(r=0.350,P=0.001;r=-0.359,P=0.001)。结论 弱视眼视网膜黄斑的结构存在异常;矫正视力、眼轴长度、屈光度会对黄斑厚度的测量结果产生一定的影响。
Abstract:
 Objective To measure the macular thickness of amblyopic and normal eyes in children with spectral domain optical coherence tomography, and provide the basis to discuss the outer mechanism of amblyopia. Methods Sixty subjects with amblyopia ( 80 eyes , amblyopic group) and 80 emmetropic children ( 80 eyes , emmetropic group) were included in this study, the ocular axis and macular parameters of two groups were measured by spectral domain optical coherence tomography, and the difference was analyzed. Results The thickness of macular fovea ( 242. 41 +22. 77) pm and outer ring of amblyopic group were thicker than those of emmetropic group ( P = 0. 048 , 0. 000 .0. 005 .0. 000 .0. 001 ) . The difference of inner ring , average macular thickness/volume [ (282. 98 +23. 05) Vm and (9. 96 +0. 73 ) mm3 ] showed no statistical significance between two groups. There was no statistical significance in macular parameters between boys and girls in amblyopic group ,while in emmetropic group ,boys presented to have thicker macular thickness than girls in the nasal, temple , inferior quadrant of the inner ring ( P = 0. 03 ,0. 01 .0. 02 ) . There was no statistical significance in macular parameters among different levels of amblyopia ( all P > 0. 05 ) . In amblyopia eyes , there was correlation between thicknesses of the inferior quadrant of the inner ring and corrected visual acuity ( r = 0. 267 , P = 0. 017 ) . And there was significantly correlation between thicknesses of the nasal quadrant of the outer ring and spherical equivalent refraction. axial length ( r = 0. 350 ,P = 0. 001 ;r = - 0. 359 .P = 0. 001 ) . Conclusion Thickness alteration may be associated with amblyopic disorders in young patients. And axial length , spherical equivalent refraction are related with macular thickness.
