摘要:
 目的　评价活血化瘀中药制剂银杏叶提取物、灯盏花素注射液治疗非动脉炎性前部缺血性视神经病变（ｎｏｎａｒｔｅｒｉｔｉｃａｎｔｅｒｉｏｒｉｓｃｈｅｍｉｃｏｐｔｉｃｎｅｕｒｏｐａｔｈｙ，ＮＡＩＯＮ）的疗效。方法　选取我院住院治疗的ＮＡＩＯＮ患者５０例，随机分为三组，均给予针灸、复方樟柳碱穴位注射等基础治疗，银杏叶提取物组增加银杏叶提取物注射液８７．５ｍｇ每天１次静脉滴注，灯盏花素组增加灯盏花素注射液１００ｍＬ每天１次静脉滴注，对照组不予中药注射剂治疗，各组均连续治疗２周，以治疗前后患者最佳矫正视力、视野平均敏感度（ｍｅａｎｓｅｎｓｉｔｉｖｉｔｙ，ＭＳ）的改变作为疗效评价依据。结果　银杏叶提取物组治疗前后视力分别为３．６５±１．５０及３.９５±１．３９，灯盏花素组治疗前后视力分别为３．８９±１．２４及４．０２±１．１９，对照组治疗前后视力分别为３．６９±１．５２及３．８１±１.５７，其中银杏叶提取物组视力提高幅度最大，银杏叶提取物组和灯盏花素组治疗前后视力比较均有显著差异（均为Ｐ＜０.０５），对照组治疗前后视力比较差异不明显（Ｐ＞０．０５），三组患者治疗后视力改善幅度之间无显著差异（Ｐ＞０．０５）。银杏叶提取物组和灯盏花素组治疗后ＭＳ高于治疗前，其中灯盏花素组治疗后ＭＳ提高幅度最大，治疗前后比较差异有统计学意义（Ｐ＜０.０５），而对照组治疗后ＭＳ反而有所降低，但差异不明显（Ｐ＞０．０５）；银杏叶提取物组有效率为４５．２％，灯盏花素组总有效率为４６．７％，对照组有效率仅为２６．７％，组间比较差异均无统计学意义（均为Ｐ＞０．０５）。结论　活血化瘀中药在ＮＡＩＯＮ的治疗中有较好的应用前景。
Abstract:
 Objective To evaluate the curative effects of ginkgo biloba extract injection and breviscapines injection on nonarteritic anterior ischemic optic neuropathy ( NAION) . Methods Fifty NAION patients were randomly divided into three groups, group A treated with ginkgo biloba extract injection, 87. 5 mg once per day, group B treated with breviscapines injection , 100 mL once per day , and group C with no TCM injection treatment, each of the patients from 3 groups were treated with acupuncture, compound anisodine and ohter supporting treatment. Best corrected visual acuity ( BCVA) and mean sensitivity ( MS) of visual field were detected before and after 2 weeks continuous treatment to evaluation the efficacy. Results BCVA of group A at initial and after treatment were 3. 65 + 1. 50 and 3. 95 + 1. 39 ,while group B were 3. 89 + 1. 24 and 4. 02 + 1. 19 . and group C were 3. 69 + 1. 52 and 3. 81 + 1. 57. All the three groups were higher than before treatment .the difference were significant of group A and group B (P <0. 05 ) .but not significant of group C (P > 0. 05) . But the difference of BCVA improvement were not significant among three groups (P > 0. 05 ). MS of group A and group B after treatment were higher than before . the difference in group B was significant (P > 0. 05 ) . While the MS of group C after treatment has decreased, although there was no sigruficant difference (P > 0. 05 ) . The total effective rate based on the eyes number of the three groups were 46. 7% ,45. 2% ,26. 7% ,respectively , there was no statistical difference among the three groups ( all P > 0. 05 ). Conclusion TCM injection by activating blood circulation of NAION patients may have a good application prospect in the future.
