摘要:
目的 使用Pentacam探讨3种近视激光矫正手术对角膜后表面的影响。方法 选取2012年1月至2012年12月来我院近视激光治疗中心行近视矫正手术患者66例进行研究,其中IntraLase飞秒激光组24例47眼,LASIK组20例40眼,LASEK组22例43眼,使用Pentacam测量各组患者术前及术后1个月、3个月、1a时角膜后表面高度及曲率变化,并测量术前眼压、中央角膜厚度(centralcornealthickness,CCT)和术中切削深度(ablationdepth,AD)。结果 术前3组患者球镜度数、散光度数、CCT、AD及眼压之间的差异均无统计学意义(均为P<0.05)。术后1a时飞秒激光组、LASIK组、LASEK组角膜最薄点高度分别为(4.13±2.25)μm、(4.35±2.29)μm、(4.00±2.41)μm;角膜顶点后表面高度为(1.30±0.91)μm、(1.25±0.89)μm、(1.12±1.08)μm。角膜最薄点高度术前及术后各时间点3组之间的差异无统计学意义(P>0.05,重复测量方差分析),角膜顶点高度各个时间点的差异亦无统计学意义(P>0.05)。术后1a时3组患者角膜后表面曲率分别为(-636±026)D、(-6.38±0.24)D、(-6.33±0.23)D,3组患者之间角膜后表面曲率比较,差异无统计学意义(F=0.213,P=0.808)。结论 飞秒激光制瓣LASIK、板层刀LASIK、LASEK术后角膜后表面高度和曲率均没有发生明显变化,角膜后表面形态均相对稳定。
Abstract:
Objective To investigate and evaluate posterior corneal changes after three corneal refractive surgery as measured by Pentacam. Methods Sixty-six patients underwent refractive surgery were recruited from January 2012 to December 2012,including 47 eyes in 24 patients in Femtosecond group,40 eyes in 20 patients in LASIK group and 43 eyes in 22 patients in LASEK group. The change of posterior corneal elevation was measured by Pentacam at preoperative time, postoperative I month, 3 months , 1 year. Additionally , preoperative intraocular pressure ( IOP) , central corneal thickness ( CCT) and ablation depth ( AD) were measured. Results There was no sigruficant difference for preoperative spherical refractive, astigmatism, CCT, AD and IOP among the three groups ( all P < 0. 05 ) . After one year , the posterior elevation of the thinnest point in the Femtosecond group , LASIK group and LASEK group were (4. 13 + 2. 25 ) Vm , ( 4. 35 + 2. 29 ) ym , ( 4. 00 + 2. 41 ) Vm , respectively. The data of the central point were ( 1. 30 +0. 91) ym , ( 1. 25 + 0. 89) ym , ( 1. 12 + 1. 08 ) ym , respectively. Change in posterior elevation of the thinnest point before and after surgery at any time point had no significant difference within the three groups ( P > 0. 05 , repeated measurement one-way ANOVA). There was also no significant difference at the central point ( P > 0. 05). The posterior corneal curvature was ( - 6. 36 + 0. 26 ) D, ( - 6. 38 + 0. 24 ) D. ( - 6. 33 +0. 23) D in the three group after one year. There was no difference before and after surgery in posterior corneal curvature within the three group ( F = 0. 213,P = 0. 808) . Conclusion There is no change in posterior corneal elevation and curvature after Femtosecond laser . LASIK and LASEK. and the posterior corneal surface is stable.
